Seasonal Variation of Group a Streptococcus (GAS) – Related Necrotising Fasciitis Cases in a UK Teaching Hospital

چکیده
Objectives: Necrotizing fasciitis is a diverse syndrome primarily involving the fascia but also occasionally the overlying skin and underlying muscles. Non-necrotising lower limb cellulitis has been reported to have a marked seasonal variation in our area. We are describing the seasonal variation of our necrotising fasciitis cases. Methods: The medical records of all the patients that were admitted to our institution with a diagnosis of necrotizing fasciitis, from January 1994 to December 2003, were retrospectively reviewed. Results: The necrotising fasciitis infections peaked every year in the spring time, as previously described for cellulitis. The increased incidence was only apparent for group A streptococcus related necrotising fasciitis cases. The diagnosis carried 21% mortality during the same admission. Overall, 39% of the patients had a bad outcome, defined as either death during the same admission, limb amputation or severe disability. 73% of patients who experienced a bad outcome were found to have Group A streptococcus compared to only 41% of the patients in the good outcome group. Conclusions: A seasonal variation was noted for Group A streptococcus associated necrotising fasciitis, that remains a lethal condition. As patients with Group A streptococcus associated necrotising fasciitis tend to have a worse outcome in our series, increased awareness and targeted antibiotic therapy may enhance clinical outcomes. Knowledge of microbiology might also inform infection control decisions.

AIMS To describe the morbid anatomical and bacteriological features in a series of five cases of rapidly fatal Streptococcus pyogenes necrotising fasciitis. METHODS Post mortem and bacteriological examinations were made of five patients dying within 48 hours from rapidly fatal necrotising fasciitis. RESULTS All five cases died rapidly from a toxic Streptococcus toxin syndrome as a result of...

A 3-day-old baby girl developed septicaemia, meningitis, and necrotising fasciitis due to group A beta-haemolytic streptococcus, type M52, which was also cultured from the mother's cervix. Necrotising fasciitis is a severe infection of the skin and subcutaneous tissues with infarction, necrosis, and sloughing of the affected areas. Early recognition of this condition is essential so that approp...

Introduction The term necrotising fasciitis (NF) was first coined in 1952 by Wilson1 to describe a rapidly progressive inflammation and necrosis of subcutaneous tissues and the deep layer of superficial fascia with sparing of the deep fascia and muscle. It had previously been described variously as haemolytic gangrene, acute streptococcal gangrene, gangrenous erysipelas, necrotising erysipelas,...

BACKGROUND Since the mid-1980's there has been a worldwide resurgence of severe disease from group A streptococcus (GAS), with clonal clusters implicated in Europe and the United States. However GAS associated sepsis and rheumatic fever have always remained at high levels in many less developed countries. In this context we aimed to study GAS necrotising fasciitis (NF) in a region where there a...

INTRODUCTION Group A streptococci is the causative agent in 80 percents of human streptococcal infections. The only member of this group is Streptococcus pyogenes. CLINICALFEATURES OF GAS INFECTIONS: The various clinical entities and related complications caused by pyogenic streptococci are reviewed in the article. Pharyngitis, scarlet fever, skin and soft tissue infections (pyoderma, celluliti...
